This Medical Consent describes important information about telehealth services, provider review, prescriptions, compounded medications, and your responsibilities as a patient or prospective patient.
Telehealth Consent
By using the Peptela website, submitting an intake form, creating an account, or continuing with provider review, you consent to receive services through telehealth when available and appropriate. Telehealth may include online intake forms, asynchronous review, electronic communications, phone or video communication, medical record review, treatment recommendations, prescription decisions, refill review, and follow-up communications.
Provider Relationship
Submitting an intake form or completing checkout does not automatically create a provider-patient relationship and does not guarantee treatment approval. A provider-patient relationship, if established, is determined by the licensed provider or provider network involved in your review.
No Emergency Care
Telehealth services through this platform are not for emergencies. If you are experiencing a medical emergency, call 911 or seek immediate in-person medical care.
Your Responsibilities
You agree to provide accurate, complete, and current information, including medical history, medications, allergies, conditions, prior treatments, and relevant health updates. You understand that inaccurate or incomplete information may affect provider review and treatment safety.
Risks and Benefits
All medical treatments involve potential risks, benefits, side effects, and limitations. You should ask questions and review any medication instructions, warnings, contraindications, and provider guidance before beginning treatment.
Compounded Medications
Some medications may be compounded. Compounded medications are not FDA-approved and are not evaluated by the FDA for safety, effectiveness, or quality. Prescribing decisions are made by licensed providers when clinically appropriate.
Pharmacy and Fulfillment
If prescribed, medications may be dispensed by licensed pharmacies or pharmacy partners. Processing and shipping timelines may vary based on provider review, pharmacy availability, shipping carriers, and other factors.
Communication Consent
You consent to receive communications related to your intake, provider review, account, treatment status, order updates, refill reminders, support, and related services by email, SMS, phone, or other available communication methods.

The assessment made available on the Peptela website does not create a doctor-patient relationship between the individual completing the assessment and Peptela. Peptela, a network of US-licensed doctors that adhere to rigorous medical protocols designed for patient safety, has established exclusionary criteria to determine if an individual does not qualify for GLP-1s. The answers an individual provides to the Peptela assessment consequently determine if the individual is screened out of eligibility for GLP-1 medication, and a Peptela clinician will meet with an individual after checkout to determine if they qualify for a prescription. Peptela clinicians retain the decision to prescribe compounded GLP-1s to patients. Compounded GLP-1s are produced in FDA-regulated facilities. Although these facilities are highly regulated, the medications are not FDA-approved or evaluated for safety, efficacy, or quality. The decision to use compounded drugs is guided by the licensed provider’s medical judgment, which is informed by a telehealth consultation and medical history. Peptela does not produce compounded medications, and individuals may receive medication that looks different than what is portrayed on the website.
